This is the whole instrument. No employer sees an individual answer, and no question in it screens anybody for a condition. If anything here would not sit well with your workforce, better to know now than after it has gone out.
Twenty four questions. About five minutes. Every answer is a score or a choice from a list, and there is nothing to type anywhere in it, because written comments identify people.

Not collected at any point. Names. Email addresses against answers. Staff numbers. Job titles. Departments. Free text of any kind.
Reported. Group figures for the whole organisation. Three cuts and no others: length of service, whether somebody manages people, and how they work. Each cut is reported one at a time and never crossed with another, because crossing two or three produces groups small enough to point at a person even when each one on its own does not.
Floors. No figures at all below thirty replies. No cut reported where either side is below thirty. Nothing shown below five in any cell.
